Yes, if you have a YRMC CareConnect account you can access your health record online.

 

It is convenient to register on-line.  You will be asked to provide your first and last name, date of birth, medical record number and other information that will help us identify you in our medical records.  If you require assistance or have any questions, please contact the Health Information Management Department.

 

Absolutely! When you register for care at YRMC East or YRMC West, a member of the Admitting Department will ask if you would like to subscribe to YRMC CareConnect. You’ll need to provide an e-mail address. You’ll also be asked for a unique security question and an answer to the security question. We understand that it may not always be appropriate to discuss YRMC CareConnect during your registration process. Because of this, a representative from YRMC’s Admitting Department may visit you during your stay to tell you more about CareConnect.

 

Parents and legal guardians may register and create YRMC CareConnect accounts for their children up to 12 years of age.

Resetting your personal password is easy. Visit the YRMC CareConnect sign-in page and go to “Forgot your Password.” Follow the instructions to re-set your password. If you’re still having difficulty signing on to the website, call 1-877-621-8014.

 

While HIPAA law does permit providers to charge, YRMC does not charge patients, their family members or other healthcare providers to copy health records. All others are charged copying fees based on Arizona state guidelines.

 

Yes, we can provide certain documents on CD.

Due to confidentiality reasons, we cannot fax health record copies to homes or businesses. We can, in most instances, fax them to other healthcare providers.

 

You may pick up your health record copies during regular business hours (8:30 am - 5:00 pm). For your protection, we require photo identification before releasing copies of your health record.

 

Yes. If the information is complete and available, and your request is manageable, we may be able to complete your request while you wait.

 

To protect your privacy, another person may not receive copies of your health record without a written consent that authorizes the YRMC HIM Department to provide copies. We also need a photo ID.

 

The executor of the will may request records of a deceased patient. If there is no executor in place, Arizona allows a deceased patient’s spouse, adult child, parent of deceased, adult sibling of deceased, or guardian/conservator may obtain the deceased patient’s health record. You may be asked to provide documents to validate proof of patient status and your relationship. The state allows YRMC to release these records in the following priority order: personal representative of estate, spouse, acting trustee, adult child, parent, adult sibling, and guardian.

 

A CD of X-ray images can be obtained by contacting the HIM Department and submitting a signed release.

As a patient of YRMC PhysicianCare, you have several options to obtain copies of your medical records. You may establish a YRMC CareConnect account to have on-line access to your records. You also may contact the YRMC PhysicianCare medical clinic where you receive care or contact the HIM Department.
