The executor of the will may request records of a deceased patient. If there is no executor in place, Arizona allows a deceased patient’s spouse, adult child, parent of deceased, adult sibling of deceased, or guardian/conservator may obtain the deceased patient’s health record. You may be asked to provide documents to validate proof of patient status and your relationship. The state allows YRMC to release these records in the following priority order: personal representative of estate, spouse, acting trustee, adult child, parent, adult sibling, and guardian.
