Yavapai Regional Medical Center’s (YRMC’s) Total Healing Environment recognizes the relationship between the body, mind and spirit. Our Care Management team puts this philosophy into action by helping patients and their families prepare physically and emotionally to return home after hospitalization or move to the next level of care.

Formerly known as Social Services, our Care Management team includes experienced registered nurses, social workers and support staff who work with patients and their families throughout their hospitalization. In partnership with patients and their families, our Care Management team:

• Evaluates post-discharge needs to help patients safely transition to the next stage of care
• Screens for potential behavioral risks
• Recommends assistance to address behavioral concerns
• Identifies community resources to address medical, financial, legal and social services
• Serves as an advocate by interacting with health insurance providers on the patient’s behalf
• Offers bereavement support and counseling
• Participates in the preadmission process in the Emergency Departments at YRMC East and YRMC West in order to ensure an appropriate admission status for patients
